Question: A 58-year-old woman presented with a 3-month history of recurrent hematochezia, a 2-week history of necrosis on the dorsum of her right foot, and intermittent fever during the past half-month, with temperatures reaching up to 38°C. She had previously been diagnosed with severe ulcerative colitis (UC), which was initially treated with mesalazine and glucocorticoids without significant improvement. After 2 infliximab injections, her symptoms continued to progress, and she discontinued the medication on her own.
