We read with great interest the article by Olivera et al1 addressing the elevated risk of Crohn’s disease (CD) in first-degree relatives (FDRs) from multiplex families compared to simplex families. The study elegantly combines genetic, microbiome, and inflammatory biomarkers to provide a comprehensive view of the risk factors associated with CD. While the findings are significant, a deeper analysis of the interplay between genetic susceptibility, subclinical inflammation, and environmental factors is warranted, particularly given the complexity of CD pathogenesis.
